Live Streaming to Flash Media Server

So I have installed Flash Media Server (Adobe) on one of my servers.

What I need to do is stream to it from my home.

And be able to play the stream though a RTMP player(going to use JW Flash).

What are the URLs I need to put in Live Encoder and JWflash player?
