They were a couple of pics back when he took leave that were fake. But it seems kind of weird he wouldn't but clothes to fit him better. I know if it's real he might just not care but he is rich and surely if he's out in public he'd probably dress up a little nicer in order to not worry anyone. That's just my opinion. Hope they're fake too. ;(

It's possible but not likely. If you look at Steve Job's last keynote it was WWDC 2011 and he didn't look that bad during the keynote. I don't think there would be that much of a difference in his condition from then to now. You can see what he looked like at WWDC 2011 here.

I think it might be real. Which is sad. I'm hoping it's fake. There is a couple things that point to it being a fake photo.

It is definitely possible that the picture could be fake, but no one knows for sure.

It could be real but honestly if it is fake it wouldn't be the first time TMZ made something up to get views.

Regardless of the Authenticity of the picture I do hope Steve gets better.

wow that just doesn't look right. I hope he's OK.

I wouldn't be surprised if it's fake but I mean that's some pretty good photoshopping skills there.
