Apple.com, on the 15th, but it took all day and I didn't receive an email confirmation for over 25 hours. I also pre-ordered my spouse's phone (not in the same purchase session, but within hours of pre-ordering my own,) and he received a confirmation within several hours.

I believe you can't pre order from anywhere other than apple at the moment... No I haven't ordered mine, I'm going to wait until my current contract runs out and get the new handset as a free upgrade.
